What is this form?
This form collects information from mental health treatment training organizations interested in being listed on the Washington State evidence-based practice reporting guide (EBP Reporting Guide) for children’s mental health.
Why would I want my organization to be listed in the guide?
The Washington state Medicaid children’s mental health system collects information from providers and insurance payers about the use of evidence-based practices in routine children’s mental health care. Mental health agencies can only report evidence-based practices that are listed in the EBP Reporting Guide. Having your training organization listed in the Guide allows mental health agencies to “get credit” for implementing the clinical practices covered in your training.
